Services: The scope on my services would include but not limited to:
1) Monthly Group Sessions for newly diagnosed chronic kidney disease (CKD) and others with chronic disease states.
2) Initial Assessment and Group Sessions.
3) Follow-up 3-6 months depending of the individual needs of the patient.
4) Diet Education for New Transplant post-Transplant patients.
5) Medical Nutrition Therapy per provider recommendation.
6) Continuing Education Units (CEU’s) for Registered Dietitian Nutritionists.
Nutrition I.A.M. LLC is designed to provide nutrition counseling to those individuals diagnosed with chronic disease states. Dietary counseling for this patient population must be individualized to address each individual concern. As a Registered Dietitian Nutritionist, I would provide dietary counseling and assist in determining each client’s individual nutritional needs and develop a dietary regime that will assist the client in meeting those dietary needs.
